Not sure if I can pinpoint what or why it is happening but when I set up a series to record it seems to skip an episode.
When I review the series I will see episode 3 and 4 recorded but not 1 and 2. Or 1 recorded, 2 skipped and then 3 and 4.
Why this is strange is that on another DVR I will set up the same recordings as a back up and they all record.
The Flex 4K is on outdoor antenna. Fire 4k 2nd gen stick. Other DVR on an indoor antenna.
Has anyone else seen this issue?

Assuming you havent rebooted the tuner, is there anything in its logs at that time (note times in logs are utc). Where is your recording engine (flex itself,
NAS, pc....)
Number 1 cause of such (assuming episode has never been recorded) is usually not enough tuners when the recording needs to kick off..
Another thing I've seen a lot of is a pc that isn't always on being configured as a recording engine and having more space actually do the recording, then it won't show up when it's off.
You also mention using an outdoor antenna for the system that fails and an indoor antenna system that works. That could be a good indication that your signals are too hot. if I ever see 100% signal strength on any channel, I would start there and make sure there is an appropriate lte filter in place as when it states 100% you dont know how much above 100% it is and too strong a signal (on any frequency, not just the one being recorded ) can overload the device.and the noise can prevent it from working when expected.

Is this a newly aired series? Or rather, what series is it?

Professor T on PBS. It recorded all 6 episodes except for #2. It also did this with another PBS series.
Then Shark tank it missed Season 17 episode 1 and now has episodes 2 and 3 scheduled.
I have 4 tuners and only PBS being recording at the time.
No amps on antenna.

I am thinking maybe it was due to a guide issue? No way to verify that now.
