I just finished an upgrade and reinstall from a MacBook Air M1 to MacBook Pro M4 (Pro), going from Sonoma to Sequoia in the process. Using the latest HDHomeRun app from the App Store, I found that I was getting regular and constant flickering that I can only describe as brightness flickering. When it's happening, it's very noticeable on video that includes solid backgrounds. It can take a minute or two to start, but then remains constant. This effect is visible on both 720p and 1080i channels. This happens on both the laptop's display as well as an external Dell 4K monitor.
As a workaround, switching the video quality setting from high to medium appears to make the flickering effect go away. I originally assumed that setting was synonymous with deinterlacing quality, but I guess there's more to it than that.
Happy to provide any info I missed, but it's very easily reproducible on my M4 and Sequoia combination here.

Apparently I'm seeing this with video quality set to medium, as well. It seems to take longer.
