Getting "Timeout Error" on the app and Plex only records audio

I'm having problems getting my HDHomeRun EXTEND working after a couple of years in storage.

I can connect to its admin interface via my browser and the EXTEND updated its firmware with no problems.

It scanned my cable connection (Verizon FiOS) and set up all of my clear local channels without any trouble.

But I can't get it to _do_ anything. When I try to use the desktop app to stream live TV, I get "Unable to play channel: Timeout error." Plex DVR tunes and records shows happily, but it never gives me any video: the result is just an MP3 file of the show's audio.

The EXTEND's device logs don't appear to contain anything helpful. Every attempt to stream through the Mac app looks like this:

And the admin interface says that neither of the 2 tuners are in use.

Any ideas about what I should try, before I commend my soul to God and dive into my FiOS modem/router settings?
