The DVR engine might be using a stale channel list. To that point, the OP should make sure they are running the latest record engine. There was an anomaly where the previous version would not pick up changes from the channel list until the record engine was restarted (so if one installed the record...

"Well, there's your problem": No OOB Lock. It is likely if you looked at that same page at another time the frequency would be different, as the system is scanning for a valid OOB signal. Without lock the CableCARD is not going to finish initializing. Was there a power outage before the issues start...
