I would think that just scanning a Dual in HDHomeRun Setup with the coax unplugged would wipe the data on the server, and since there's no background scan on a Dual, it won't come back. I don't have a Dual that is easily accessible to test though.

Do you see it on the built-in tuner on a TV? Seeing it across multiple devices likely indicates that it's being encoded like that by the broadcaster.

You have an old HDHomeRun Dual on your network that hasn't been rescanned since sometime before the repack, and it still has old channel locations. Go into the HDHomeRun Setup program, click on the Digital Antenna tab, select the Dual, and run a channel scan to update the stored lineup data.

WMC uses RTSP for streaming from CableCARD devices. Most of the firmware is common across all modern devices, so it's probably just there. AFAIK there's no way to actually tune channels using it, as WMC uses the DRI CAS:SetChannel interface to do that and just uses RTSP for the network part of things.

Some cable providers include additional data on the data stream that contains the tables that tell the receiver where to find the audio and video data associated with a particular channel. Search for the show on the Discover tab and select it and it will give you a list of all the upcoming episodes including episode numbers, and you can schedule recordings from there.

Our system pulls in updated data from our guide provider each day. If they happened to add the listing for that channel in your area then it might update around that time. It would also be updated if someone else on the same cable provider had opened a ticket and asked us to fix it.

Do you see a Video Quality option in the gear menu in the app on the Max? What is the device ID for your HDHomeRun? We can update the listing for that channel.
