You would either need to set the speaker configuration in the HDHomeRun app to passthrough or connect the PC directly to the receiver via HDMI. S/PDIF is bandwidth-limited and can only carry DD, DTS, or max 2 channels of PCM audio.

The box is doing the decoding. It does not explicitly list an AC-4 decoder, but instead the app has to send it the audio as if it was going to do passthrough and then it decodes. This is common in devices based on the Amlogic SoCs. It should provide 5.1 where available.

There is no customer-visible diagnostic mode, but if you enable the sending of diagnostic data in the gear menu HDHomeRun app, watch an ATSC 3.0 channel, and reply back with the device ID for your HDHomeRun, we can check the logs to see what AC-4 mode is in use.
