Been doing a lot of testing lately. Here is the result.
Using the MAC for storage vs USB drive didn't really make any difference. So currently on USB Drive.
Purchased a ROKU Ultra and put on same TV as the Apple TV 4K.
The Apple consistently gets out of sync with video and audio both on live TV ...

I'm gonna try using the MAC for awhile and see if that makes any difference.
Is there any way to copy a couple recordings I have on a USB drive that was connected to the Flex to the MAC? There are a couple I haven't watched yet and would have to plug the USB drive back in to get them otherwise.

Been playing around with this for some time. As long as I use the ROKU for playback I am not having any issues.

OK, it's definitely something to do with the Apple system. I tried 3 different ROKU's, 3 different TV's, including the one the Apple is on. In all cases the ROKU worked fine. No issues with A/V sync. But the Apple is consistently getting off.
Any Thoughts from anyone?

Before going any further on this I made a couple observations. Maybe there might be some suggestions here.

Regarding this whole issue, here is what my problem is.
When playing back a recording the audio and video will get off during the playback. I can exit, restart, and it will be fine for awhile.
