So it sounds like the moral of the story is SD is working properly, the guide data provider is working properly, but the local stations are providing bad data. Any of the 'fixes' discussed are only band-aids to mask that problem, and will not work if the original broadcast is indeed preempted for so...

The problem is the first airing tag is being used incorrectly. Who sets this? Is this directly from the broadcaster? From the data provider? SD?

Are you looking at the extracted data after the TS packet headers have been stripped or the raw file data in a hex editor? Wondering if you're just seeing the sync byte for the next packet. To add some more data points, my DVR recorded S17E16 twice from the same channel. Once on 3/28 when the original showing was on, then again on 3/30. Mine records that show twice on the same channel occasionally, so removing secondary channels may not even help. We're at the mercy of the guide data and how SD determines what to record in the cloud.

I see the same thing. This is likely the result of the provider marking them all as first run when that is clearly not the case. You can verify this by checking the metadata in the recordings. The DVR doesn't apply any additional smarts to figure out that the show is indeed the same by the episode n...

It completely depends on the signal level and equipment you have on what you can get away with. I have Comcast with fiber to the house, which means RFoG and only about 50' of copper.
